Following an outbreak in the United States, the population is cut off from the outside world and the military decides to resort to bio-containment, including sending in people called Necros to enact a 'dirty war' in the city.

The film opens in the year 2022, with a group of Necros, led by a scientist, on a mission to recover a virus from a hospital. Meanwhile, a group of survivors from the original 1968 film have been holed up in a shopping mall. Due to the presence of two million Undead, the mall has become a squalid hellhole, and it is clear that the Necros are preparing a final assault against the living.
One of the members of the group, a young man named Ving (whose face and body were badly burned in the original film) is in despair. Though the group still have hope, their chances of survival are slim. During a briefing with a police officer, Ving calls out to his mother. As the officer turns away, Ving shoots him dead. He then looks through the dead officer's armoury and finds an automatic handgun. He then uses the cover of a nearby restaurant to sneak out of the mall.
Later, two survivors, Sean (the sole survivor of the original 1968 film) and his friend Nikki, meet to discuss the latest situation. Sean reveals that he is now in a relationship with Nikki's friend Anna, who is pregnant with his child. They discuss possible ways to escape the mall. They are then interrupted by a group of Necros, who have followed Sean. There is a fierce battle between the two sides, during which Nikki is badly wounded. The Necros leave, taking Nikki with them.
Sean and Anna run into another survivor, Dave. Dave has also survived the original 1968 outbreak and is still recovering from his injuries. He tells them that he is a medic, and offers to help Nikki recover.
